Just posted a short news piece to the site about the RSD packaging mix ups. It includes the copies that were packaged wrong, with the insert showing instead of the label of the vinyl which affected about 10% of the copies. It also features some information about the three known copies which shipped with a Tool EP instead of the second U2 disc.

Nice investigative work. Based on how much Twitter/internet attention it got, I though the Tool thing was more widespread.

No it's literally three copies that are known about and maybe a few more that aren't known. I was surprised at how much attention this got as well. The girl who first posted about it on twitter brought it to my attention on Saturday and Harry and I were talking back and forth with her thoughout the day. She's had to protect her tweets now because of the attention overload. She's had a few nice offers to sell it - but so far seems more interested in keeping it.
PS. I'm going to pull some of the stuff off of blogger and into that format and put together a little news section on the site to keep us going till the new site is ready.

I have heard of this happening before in American pressing plants that created punk and indie records in the late 80's and early 90's. Often reported as accidents, they were in fact anything but. Bands later admitted this.
Some pressings that were colour vinyl were different colours because of lack of materials and different shades were printed over the scale of the batch. Also sometimes another press would happen because they ran out of materials and then 're-appear' at a later date.
There is presumably an uber collector that will find value in this mispresses but personally I wouldn't want it. Personally, I would sell it, donate a % to charity and by the proper one.
Lets face it, the world of collecting seeks a good mystery and thats why these accidents happen.
